Summary. This article reviews literature regarding peculiarities in the management of pregnant rheumatic patients, and also effectiveness and safety of antirheumatic therapy during pregnancy and lactation. Disease activity of pregnant patients may increase, and it is a risk factor of adverse events for the mother and future child, and requires appropriate treatment correction. Management of rheumatic diseases during pregnancy and lactation is limited to few relative safe medications, and is based on a tight cooperation between rheumatologists, obstetrician-gynecologists and if necessary other physicians.
