Features of proatherogenic disorders in patients with systemic rheumatic diseasesГарміш О.О., Коваленко В.М., Талаєва Т.В., Третяк І.В. Summary. The aim of the work which was carried out was the determination of the proatherogenic metabolic peculiarities in 98 patients with the systemic rheumatoid diseases. Materials and methods. In all patients, the level of CRP, the content of MDA in circulating monocytes, in blood plasma and catalase activity were determined. The presence and severity of pro-atherogenic status was judged by the content of modified LDL and VLDL in the blood, which was determined by the bioassay method using peritoneal mouse macrophages. The blood content of the CIC was determined; the immunogenicity of modified Lp was determined by the content in the CIC of cholesterol and triglycerides. The spectrum of lipids and lipoproteins in the blood was evaluated in detail with an additional determination of the plasma level of apoB and apoA-1, carbohydrate metabolism disorders by the content of glucose and HbA1c in the blood were determined. Results. The obtained results show the existence in the examined patients of significant systemic inflammation in conjunction with the distinct proatherogenic metabolic state which was revealed by lipoprotein modification with the appearance in them auto-antigenic properties. These changes coincided with the moderate changes of glucose metabolism and appeared in spite of the absence of significant traditional atherogenic risk factors. The results of paired correlative analysis showed the existence of strong dependence between indexes of systemic inflammation, proatherogenic and immunogenic lipoprotein modification. Conclusions. The obtained data point that virtually all the patients with systemic rheumatoid diseases must be regarded as the persons of high cardiovascular risk independently of the stage of the main process, the character of the lipid and lipoprotein metabolism disturbances.
