Osteopenic syndrome and gastroesophageal reflux in patients with rheumatoid arthritis — an example of Clustering of Extraarticular Manifestations ConceptZazdravnov A.A. , Pasiyeshvili L.M. Summary. Summary. The relationship between the system osteopenic syndrome (OPS) and gastroesophageal reflux (GER) in patients with rheumatoid arthritis (RA) have been considered in the article. 149 patients with RA, of which 99 had evidence of GER (study group) and 50 patients with RA with no signs of esophageal disease (control group) were examined. In all cases, GER was diagnosed no earlier than one year after the diagnosis of RA. Statistical analysis of the results of clinical and instrumental examination revealed a positive association between extra-articular manifestations of RA — GER and OPS. It was found that the presence of GER in patients with RA, an increase of frequency OPS, aggravated disorders of bone mineral density. It wasfound a direct correlation between the severity of esophageal pathology and OPS on the one hand and the degree of RA activity — on the other. Dynamics of pro-inflammatory cytokine tumor necrosis factor — alpha and universal marker of inflammation — C-reactive protein showed that the main mechanism of forming abnormal cluster of еxtraarticular manifestations «osteopenic syndrome and gastroesophageal reflux» in patients with RA is the immuno-inflammatory process.
